Under the direct supervision of a higher level supervisor or Health Information Senior Technician, Health Information Technician Trainees collect patient health information, analyze and correct chart deficiencies, code patient information for statistics, reporting, and revenue and reimbursement for services provided.
Assembles and analyzes health information for completeness, consistency and compliance with hospital regulations and JCAHO standards, assuring that all relevant medical documentation is included; learns and utilizes ICD-9-CM and CPT/HCPCS nomenclature, Ambulatory Payment Classifications (APCs), and other coding systems as experience is gained; with oversight, abstracts and codes basic medical and related data from patient medical records for less complex diagnoses, clinical and other outpatient visits, and routine medical procedures; enters coded medical data into a computer terminal (Affinity or other health information system), and selects diagnoses and procedures codes from designated computer-based coding systems, such as 3M encoder; ensures medical records compliance with applicable laws and regulations, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA), JCAHO, and other laws, regulations and procedures of the facility to which assigned; verifies that cases of communicable diseases have been properly reported to the Health Department, or reports as directed by the supervisor.
